Mend is a small practice that delivers in-depth individual care. It is the only practice in the country that combines Forensic Neuro Nutrition and Chinese medicine—it’s much more than an acupuncture treatment. For that reason, new patient appointments are limited. New Patient Paperwork
You must complete the online New Patient Intake Form before your scheduled appointment time. The form goes over your health history and takes about 20-30 minutes. All information you provide will be completely confidential. The link to the form will be provided in your account and your confirmation email after you schedule your first appointment. If you do not have reliable internet access, please call and arrange to pick up a physical copy.
Booking & Rescheduling
Once you are booked for your first appointment, the scheduling software will prompt you to create an account or sign in with a preexisting social account (i.e., Google or Facebook). Your user account allows you to book multiple appointments, manage your appointments, and change your notifications. If you need to cancel your appointment, please do so at least 24 hours before your scheduled time through your account. This way, you’ll avoid a late cancellation fee, and another patient may be treated. Please reach out if you have any trouble with access.
Before You Arrive

Please wear loose-fitting clothing for easy access to arms and legs. I have gowns available if needed; you may change your clothes in the office. Loose jeans, sweatpants, or shorts are a great option.
To make your treatment comfortable and enjoyable, please make sure to have a light meal before arriving and avoid ingesting caffeine or alcohol 2-3 hours before your session.
Acupuncture treatment can continue hours after your time on the massage table. It's best to avoid strenuous activity after your appointment to avoid diminishing effects. Please plan accordingly.
Masks are optional at this time. To avoid spreading infectious diseases, please reschedule your appointment if you're coming down with a new illness. There is no fee for canceling an appointment if you believe you may be contagious.
Severe Weather
If snow or other severe weather is forecast and the roads may be dangerous, I will reach out to you to reschedule your appointment with no late cancellation charge.
